BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014203891 DESCRIPTION A 58-year-old Kurdish woman with a history of diabetes, presented to our clinic with fever and rigour for 2 week’s duration, associated with diffuse bone pain and weight loss. Laboratory work showed pancytopenia with haemoglobin 107 g/L, white cell count 3.3×109/L (absolute neutrophil count 1.5×109/L) and platelet count 8×109/L, lactate dehydrogenase was 1227 U/L and uric acid 8 mg/ dL. The blood film showed normochromic normocytic anaemia with rouleaux formation. A bone marrow aspiration was diluted and looked like an artefact, no megakaryocytes detected, the blasts account for 40% on the available cells, and they were of acute lymphoblastic leukaemia (FrenchAmerican-British (FAB)–L2; figure 1). The bone marrow biopsy revealed diffuse necrosis, cells loosing normal staining with coagulative necrosis of marrow elements in an amorphous eosinophilic background, with a small focus of lymphoid cells and few residual megakaryocytic cells seen (figure 2). Flow cytometry revealed positive CD19, CD10, TDT, CD34 and CD79a while negative for T-cell markers. The patient received a five-drug remission induction regimen with intensive consolidation used for the patient (Larson protocol). We gave cyclophosphamide, daunorubicin, vincristine, prednisone and L-asparagines on 21 July 2013. She got complete remission with normal bone marrow aspirate and disappearance of necrosis on day 28 of her treatment. Bone marrow necrosis (BMN) is an uncommon clinicopathological condition which is diagnosed at postmortem period. BMN is mostly caused by malignancy especially haematological as acute leukaemias, relapsed Hodgkin’s lymphoma or solid tumours as gastrointestinal tract and prostate adenocarcinoma, stomach and colon cancer. Non-haematological causes as sickle cell disease, infections as tuberculosis and toxic effects of chemotherapy such as interferon α, granulocyte colony stimulating factor (G-CSF), all trans-retinoic acid (ATRA) and fludarabine. Rare causes of BMN are anorexia nervosa, haemolytic uraemic syndrome and antiphospholipid syndrome were reported. 2 Pathophysiology is unknown but could be due to marrow microvascular obstruction, cytokine release and T-cell and granulocyte activation. So by early diagnosis and good supportive treatment the prognosis is promising unlike that mentioned in the literature.

Face Bones Involvement and Relapse in a Case of Childhood Acute Leukemia
Midface bones are an unusual site for primary presentation and relapse in acute lymphoblastic leukemia. Herein, we describe a case of acute pre B cell lymphoblastic leukemia with leukemic infiltration of maxilla and bone marrow involvement. At the time of relapse, the patient presented again with maxilla involvement and the phenotype changed to biphenotypic lymphoblastic leukemia. Our case sug...
متن کاملBone marrow necrosis preceding infantile acute lymphoblastic leukaemia.
We report a case of bone marrow necrosis preceding infantile acute lymphoblastic leukaemia (ALL). Bone marrow necrosis is a rare antemortem event and has been known to be present in many conditions, notably in haematological malignancies like acute lymphoblastic leukaemia. This case was a 6-month-old Chinese boy who was referred to Hospital Universiti Kebangsaan Malaysia for further investigati...

Patients suffering from adult acute lymphoblastic leukemia are acutely ill and present most commonly with fever, pallor, bleeding, lymphadenopathy, hepatosplenomegaly and presence of lymphoblasts in the peripheral blood and bone marrow. We describe a rare presentation of acute lymphoblastic leukemia, in a young adult male who had vague and minimal symptoms with mild splenomegaly. There was seve...
متن کاملEvaluation of FOXP1 gene expression in pediatric B-cell precursor acute lymphoblastic leukemia patients at remission induction therapy
Background: Transcription factors (TFs) play a key role in the development, therapy, and relapse of B-cell malignancies, such as B-cell precursor acute lymphoblastic leukemia (BCP-ALL). Given the essential function of Forkhead box protein P1 (FOXP1) transcription factor in the early development of B-cells, this study was designed to evaluate FOXP1 gene expression levels in pediatric BCP-ALL pat...

CASE REPORT A 39-year-old man presented with a 4 month history of weight loss and a 6 week history of upper abdominal pain radiating to the back with nausea and vomiting. Liver function tests showed an obstructive picture, full blood count was normal and on computerised tomography there was diffuse enlargement of the p...
متن کاملBone Marrow Necrosis in Newly Diagnosed Acute Leukemia: Two Case Reports and Review of the Literature
Bone marrow necrosis (BMN) in acute leukemia is a rare histopathological entity at the time of initial diagnosis. However, it represents an important diagnostic and prognostic challenge. Two cases of BMN are reported: a 44-year-old patient with B cell precursor (BCP) acute lymphoblastic leukemia (ALL) and a 27-year-old man with FAB-M5 acute myeloid leukemia (AML) who both presented with bone ma...
